If you wish to join the practice please call into reception. The receptionist will give you registration forms for completion and make you an appointment with the GP. If transferring from another practice, please ensure that you have an adequate supply of medication until your appointment at the surgery.
If you are joining the Practice from outside the UK additional information will be required to complete the registration:
- Visa/Passport
- Photographic ID
- Proof of address (eg tenancy agreement, bank statement)
- Birth Certificate for children
